I have spent decades working in nursing, including acute care, emergency nursing, rehabilitation, medical-surgical nursing, and home health. I currently continue to work as an Emergency Room nurse at a major Houston-area hospital, which keeps me connected to the realities of caring for patients during some of their most difficult moments.
I hold a master’s degree with a focus in Medical-Surgical Nursing, and I am also a Certified Rehabilitation Registered Nurse (CRRN). I continue to advance my education through my Family Nurse Practitioner studies because I believe there is always more we can learn to better serve our patients.
When a patient leaves the hospital, especially someone with CHF, COPD, respiratory problems, wounds, mobility limitations, or other complex conditions, I understand how important the first few days at home can be. That is why our team focuses on responding quickly, coordinating care closely, and starting services as soon as we appropriately can.
I also believe leadership should be involved. If there is a concern with one of our patients, I want our nurses and therapists to communicate it. If a case manager needs an answer, we try to respond. If a family is struggling, we want to know how we can help.
I have also made it a priority to build a team of nurses and clinicians who are comfortable caring for patients with more complex needs.
We may not be the biggest home health agency in Houston, but our goal is simple: have the heart to serve, the clinical experience to help, and the responsiveness our patients and referral partners deserve.
Continuum Home Health LLC is committed to maintaining high standards of quality and regulatory compliance.
Our clinical services are delivered according to physician-directed plans of care while meeting applicable state and federal requirements.

Our Houston office allows us to coordinate care efficiently across these communities while working closely with physicians, hospitals, and referral partners.
At Continuum Home Health, we believe good home health care starts with being available, responsive, and ready to help when the patient needs us most.
We work closely with hospitals, rehabilitation facilities, physicians, case managers, patients, and families to make the transition home as smooth as possible. Our goal is to respond quickly to referrals and, when appropriate and staffing allows, begin care within 24–48 hours.
What makes our team different is our comfort level in caring for patients who may need more support at home. Our clinical team has experience caring for patients with conditions such as CHF, COPD, respiratory conditions, complex wounds, tracheostomies, feeding tubes, ostomies, Foley catheters, and other higher-acuity needs.
Our services include Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aides, Medical Social Services, wound care support, infusion-related care, and respiratory support when appropriate.
We understand that sending a medically complex patient home can sometimes be difficult for both the family and the discharge team. Our job is to help make that transition easier. We communicate, coordinate, educate the family, and address concerns early whenever possible.
Continuum is a nurse-led agency, and we want our patients and referral partners to feel that there is always someone paying attention.
For us, home health is not simply about completing a visit. It is about helping the patient recover safely, stay at home, avoid unnecessary hospital visits, and feel supported throughout the process.
Choosing a home health provider is an important decision. Our goal is to make the process as comfortable and straightforward as possible.
Every patient receives an individualized care plan based on physician recommendations, health goals, and personal needs.
We maintain ongoing communication with patients, families, physicians, and caregivers to support coordinated care throughout the treatment journey.
Our licensed nurses, therapists, aides, social workers, and respiratory specialists work together to deliver high-quality home health care with professionalism and compassion.
